Program Qualifications

Each program has different criteria and eligibility. Below are some of the situations that apply to most of our members:

• Employer-sponsored: When we partner with an employer to enroll their employees in our program. In that case, the employer will determine the selection and the enrollment criteria.
  
  

• Independent member: When an individual served by our team is interested in a given program. Most of the insurance plans cover the majority of our program, if not, then we will communicate that before the beginning of the program and an out-pocket fee might be required.
  
  

• Qualified member: When we identify an individual who is served by our team and might benefit from one of our programs, we might extend an invitation for them to join the program at no cost.
